Once your account has been created, we will send you an email with details on how to set up your portal account. Your portal account is where you will submit cases, check case status and make payments.

Canada Rx Sheets & Submitting a Case

To submit a case, you will need to fill out an Rx form. Please note that you MUST use a designated Canada Rx form to submit your case. We are working to offer all appliances and products to Canada, but in the meantime, the appliances listed on the below prescription sheets are available for purchase in Canada.

Submitting Your Case

Complete intraoral scan or take model/impression
- If you use an iTero scanner to submit your digital scans, please call 800-577-8767 and add the Specialty Appliances ID: 10767. If you use a different scanner, please be sure to add Specialty Appliances to your scanner as a lab. Use the email address digital@specialtyappliances.com to add us as a "partner" and ensure that your cases continue to be received.
- If you plan to ship in stone models or impressions, you are required to provide your own shipping label. Please send complete models/impressions and prescription sheets to the following address. Please note that we are not responsible for any fees or delays due to shipping and/or customs.

Fill out prescription sheet.
For digital cases, submit scan AND prescription sheet to Specialty Appliances. For analog cases, ship models/impressions AND prescription sheet to Specialty Appliances.
