MARPE 101: When, Why, and How It Works
Orthodontic treatment has evolved significantly over the years, but one of the biggest shifts has been how we approach maxillary expansion in non-growing patients.
For many clinicians, MARPE has opened the door to treating cases that previously required surgery or compromise.

What Is MARPE?
MARPE is a skeletal expansion technique that uses temporary anchorage devices (TADs) to deliver forces directly to the maxilla.
Unlike traditional expanders that rely primarily on dental anchorage, MARPE shifts the force to the underlying bone—allowing for more predictable skeletal expansion, especially in late adolescents and adults.
In simple terms: Less tipping. More true expansion.
The key advantage of MARPE is force distribution.
Traditional expanders often result in:
- Buccal tipping of posterior teeth
- Limited skeletal movement in mature patients
- Higher relapse potential
MARPE changes that by anchoring the appliance with TADs, allowing expansion forces to act more directly on the midpalatal suture.

This makes it possible to:
- Achieve skeletal expansion in non-growing patients
- Reduce unwanted dental side effects
- Improve airway and arch development potential
When Should You Consider MARPE?
MARPE isn’t for every case, but in the right situation, it can be revolutionary.
Here are a few common indications:
- Non-Growing or Adult Patients
- When traditional expansion is no longer predictable due to suture maturation.
- Transverse Maxillary Deficiency
- Patients presenting with narrow arches, posterior crossbite, or crowding driven by skeletal constriction.
- Cases Where You Want to Avoid Surgery
- MARPE can often reduce or eliminate the need for surgically assisted expansion.
Clinical Considerations
While MARPE is powerful, outcomes are highly dependent on planning and execution.
A few factors to consider:
- Patient selection matters
